Setting up an Initial Appointment
Before an initial appointment can be established, we will have to gather some information about the prospective patient and validate his/her insurance (if applicable). This paperwork will have to be completed prior to being seen for the initial consultation appointment. This can be completed at home and mailed, faxed or dropped off at our office for review. Once your initial paperwork has been submitted and reviewed you will be contacted by telephone to establish an appointment time for your initial consultation.
